That happened to me once.

My shifter handle is attached to the shifter mechanism by two bolts, and was installed without the use of locking nuts. I failed to notice it loosening up over time, and one of the bolts backed out. This allowed the handle to fall down while still being attached.

When it happened, I was in the process of downshifting as I exited the freeway, less than half a mile from the carpool lot where I was to drop off my coworker. Not having the leverage of the handle, I had to wrestle the shifter into place.

Luckily, my coworker had a small socket set in his truck and I was able to re-secure the lever in just a couple of minutes. I later got some nylon locknuts to prevent such a thing from happening again.

Oh yeah, I’ve been there, too... One of the bolts sheared that held the handle on mine. A vice-grip was too far forward for use of 1-3-5-R as it would bump into the center console.
